If you suspect that your FlexCard has been lost or stolen, do not hesitate in taking action: fraudulent charges may begin to be made within hours of a theft.
- IMMEDIATELY contact U.S. Bank at (800) 344-5696, select option number 3, “Report Lost or Stolen Cards” and proceed as directed in order to cancel your card and request a replacement. The bank will let you know if any fraudulent charges have made it onto the card (see the Fraud and Disputes section on what to do when fraudulent charges appear on your card).

Inform your Department Administrator of the lost or stolen card: they will report it to FlexCard Administration by submitting a ServiceNow Ticket
- Your replacement FlexCard, with a new account number, will be delivered to the FlexCard's billing address within 3 working days of your phone call to the bank.
IMPORTANT NOTE: If the canceled FlexCard was being used to pay for monthly charges such as Arrowhead or Verizon, you must contact the merchant and have your account switched over to the new FlexCard account number.
