Re: Updated Declaration of Missing Evidence Form Available
Business & Financial Services Accounts Payable has made revisions to the Declaration of Missing Evidence form, used with a payment/travel expense reimbursement request when an original receipt has been lost or misplaced, and has eliminated the separate Travel & Entertainment version of the Declaration of Missing Evidence form.
Payees should begin using the new combined form immediately. Effective December 1, 2019, requests for reimbursement submitted using older versions of the form will be sent back without any notification.
