Section II: Department Head Approval

Section II is completed by the Department Head directly in the downloaded PDF. It includes the approver's phone extension, date, and printed name, title and department, plus a signature after the PDF is printed or downloaded. There is nothing to enter for it on this page.

Exceptional Approval (only if this award exceeds the limits above): If the award amount exceeds the standard dollar limits, the approving official who granted the exception must also add their name and sign the Exceptional Approval line in the PDF, with the date. Leave this blank if the award is within the standard limits.

Your Section I entries will be pre-filled in a fillable PDF that downloads to your device.
